There are a few things that we need as a provider for you to complete before your first appointment.
A brand new amputee must contact their physician and get approval to be fitted with a prosthesis and obtain a prescription for a new prosthesis.
An experienced prosthetic user must contact their physician and discuss the need for a new prosthesis. Please ask for a prescription for a new prosthesis as well as a letter of medical necessity that describes the need for a new prosthesis.
Patients needing an orthotic device will also need to obtain a prescription from their physician as well as a letter of medical necessity describing why the orthotic device is needed and what it will be used for.
What should I bring with me to my appointment?
1) Your prescription or referral
2) Your insurance card
3) If you are not the subscriber on the insurance plan, please have the subscriber's information available
4) Cash, Credit Card, Check for any co-insurance or co-pays that you may have
5) For new patient's and patient's that have any changes to current information, you can print the forms below and bring them with you to your appointment
